You must complete a "Client Testimonial Submission Form". To read and print the Client Testimonial Submission form, click here.

You will need to write the "All About U" Letter about your reasons to join U weight Loss, your weight loss accomplishment, and the new U.

You will need to include the "before" and "after" photos of yourself. Full-body shot photos in which you are alone are preferred. 3 x 5 or 4 x 6 prints or digital/scanned pictures on a disk/CD are accepted.

Send the completed "Submission form", your "All About U" letter, and before/after photos to the address below. Your photos will become the property of U Weight Loss ™ Clinic and will not be returned.
